What is Phoenix Housing?
Phoenix Housing is a Shelter Plus care Program Offered by Project Concern. Shelter Plus Care is a federal program funded by the U.S. Department of Housing and Urban Developmnet (HUD) designed to provide rental subsidies and supportive services to homeless individual with diasbilities and their families.
Participants live in housing Project Concern leases from private landlords. Participants pay 30% of thier adjusted annual gross income for rent.
In order to receive this assistance, participants are required to maintain consistent and ongoing supportive services.
Who is Eligible?
To qualify for Shelter Plus Care assistance, applicants must be both homeless and disabled. Homless individuals must be residents of:
- The Dubuque Rescue Mission
- Teresa Shelter
- Hillcrest Family Services Transitional Housing *
- Maria House *
* Residents of Transitional Housing must meet the HUD definition of homelessness
A household must have at least one adult determined by a licensed professional. Preference is given to those persons disabled by
- Mental Illness
- Chronic Problems with Alcohol and/or Drugs
- HIV / AIDS
Eligible individuals/household members will be legal residents of the United States and be able to provide verifiable identification for all occupants listed in the application. Proof of homelessness, diability, and residency will be required at the time of interview.
Supportive Service Requirement
Applicants must be receiving services from on of the following services providers:
- Hillcrest Family Services Mental Health Clinic
- The Dubuque Community Y Domestic Violence Program
- Substance Abuse Services Center
Income Guidelines
Eligibility is determined by income limits set by HUD. Assets may effect eligibility.
